We are seeking a seasoned MDS Program Manager to join our dynamic team! This position plays a vital role in the care planning process by ensuring timely and accurate completion of the Minimum Data Set (MDS) assessments. This position works collaboratively with interdisciplinary teams to develop comprehensive care plans that reflect the individualized needs of each resident.
Requirements
- BS Degree from an approved school.
- Nursing license for RN and eligible to practice as an RN in the state of SC.
- Current Health Care Provider CPR Certification.
- Minimum of three (3) years experience in a hospital, nursing care facility, or other related medical facility.
- Driver’s license required with verified three (3) year safe driving record.
Responsibilities
- Coordinate and oversee the MDS assessment and care planning processes.
- Ensure accurate and timely completion of MDS assessments in accordance with federal and state guidelines.
- Collaborate with interdisciplinary teams to gather assessment data and ensure comprehensive resident care plans.
- Monitor Quality Measures and implement improvement strategies.
- Educate staff on MDS documentation requirements and support compliance.
- Participate in audits, surveys, and performance improvement initiatives.
