About The Position
Broad Reach Healthcare / Liberty Commons Rehab & Skilled Care is seeking an experienced MDS Nurse to join our interdisciplinary team. This new full-time (37.5 hours/week) position will expand our MDS department to four nurses, strengthening our ability to manage a high-volume, high-acuity case mix while ensuring compliance, timeliness, and optimal reimbursement capture.
Requirements
- Licensed RN or LPN with strong clinical assessment skills
- Minimum 1–2 years MDS/RAI experience required
- Strong knowledge of Medicare, Medicaid, and Managed Care processes
- Highly organized with excellent communication and collaboration skills
- Detail-oriented, with a commitment to compliance and quality
Responsibilities
- Complete case start-to-finish: DX, MDS, Care Planning, Case Management
- Lead and manage managed care caseloads while flexing to support LTC and Medicare MDS as needed
- Conduct timely and accurate assessments (5–6 per week typical in LTC, plus subacute/managed care)
- Collaborate with IDT to ensure compliance with federal/state regulations and QRP thresholds
- Support training initiatives (e.g., Section GG) and contribute to ongoing staff development
