About The Position
Mercer Manor Rehabilitation is seeking an MDS Nurse (RN/LPN) to join our skilled nursing facility. This is a full-time, salaried opportunity offering competitive compensation, performance incentives, comprehensive benefits, and a supportive leadership team. Previous MDS experience is preferred, but we are willing to train the right RN or LPN with skilled nursing or long-term care experience.
Requirements
- Active Illinois RN or LPN license required.
- Skilled nursing or long-term care experience required.
- Strong organizational and communication skills.

- Previous MDS experience preferred; training is available for the right candidate.
Responsibilities
- Coordinate, schedule, and complete MDS assessments.
- Ensure timely and accurate MDS documentation.
- Collaborate with the interdisciplinary team to support quality resident care.
- Help maximize appropriate reimbursement through accurate assessments.
- Maintain compliance with state and federal regulations.
