We are seeking a compassionate, organized, and experienced MDS Nurse/Nurse Manager to join our leadership team. This is a full-time, benefit-eligible position ideal for a nurse who thrives in both clinical leadership and clinical data assessments. Position Summary The MDS Nurse/Nurse Manager plays a key leadership role in overseeing resident care, supporting nursing staff and completing the MDS process to ensure accurate assessments, care planning, regulatory compliance for maximum reimbursement. This position combines hands-on clinical leadership with MDS responsibilities and requires strong communication, critical thinking, and organizational skills.

Requirements

  • Current RN license in the State of Wisconsin
  • Minimum of 2 years MDS experience required
  • CPR certification or willingness to obtain within 30 days of hire

Nice To Haves

  • Long-term care or skilled nursing experience preferred
  • Strong assessment, documentation, and organizational skills
  • Ability to lead, mentor, and communicate effectively with staff and families
  • Experience with care planning, Medicare/Medicaid documentation, and regulatory compliance preferred

Responsibilities

  • Coordinate and complete MDS assessments accurately and timely
  • Ensure compliance with State and Federal regulations
  • Participate in care plan meetings and interdisciplinary collaboration
  • Review documentation to support reimbursement and quality measures
  • Monitor clinical documentation for accuracy and completeness
  • Assist with audits, quality improvement initiatives, and survey preparedness
  • Provide clinical leadership and oversight for residents and staff
  • Support, mentor, and coach CNA and Nursing staff
  • Monitor resident conditions and communicate changes appropriately
  • Assist with assessments, care planning, and interventions
  • Ensure high-quality resident-centered care and compliance with facility policies
  • Help maintain staffing coverage and assist on the floor as needed during call-ins or staffing shortages
