We are seeking a compassionate, organized, and experienced MDS Nurse/Nurse Manager to join our leadership team. Position Summary The MDS Nurse/Nurse Manager plays a key leadership role in overseeing resident care, supporting nursing staff and completing the MDS process to ensure accurate assessments, care planning, regulatory compliance for maximum reimbursement. This position combines hands-on clinical leadership with MDS responsibilities and requires strong communication, critical thinking, and organizational skills.
Requirements
- Current RN license in the State of Wisconsin
- Minimum of 2 years MDS experience required
- CPR certification or willingness to obtain within 30 days of hire
Nice To Haves
- Long-term care or skilled nursing experience preferred
- Strong assessment, documentation, and organizational skills
- Ability to lead, mentor, and communicate effectively with staff and families
- Experience with care planning, Medicare/Medicaid documentation, and regulatory compliance preferred
Responsibilities
- Coordinate and complete MDS assessments accurately and timely
- Ensure compliance with State and Federal regulations
- Participate in care plan meetings and interdisciplinary collaboration
- Review documentation to support reimbursement and quality measures
- Monitor clinical documentation for accuracy and completeness
- Assist with audits, quality improvement initiatives, and survey preparedness
- Provide clinical leadership and oversight for residents and staff
- Support, mentor, and coach CNA and Nursing staff
- Monitor resident conditions and communicate changes appropriately
- Assist with assessments, care planning, and interventions
- Ensure high-quality resident-centered care and compliance with facility policies
- Help maintain staffing coverage and assist on the floor as needed during call-ins or staffing shortages
