About The Position

Elderwood at Hamburg is seeking an experienced MDS Coordinator (RN) to join their skilled nursing and rehabilitation team. This role is for an MDS professional who values organization, collaboration, clinical accuracy, and supportive leadership within a long-term care environment. The position is responsible for the timely and accurate completion of Minimum Data Set (MDS) assessments in accordance with RAI Guidelines, CMS regulations, OBRA requirements, Medicare and payer guidelines, and state/federal compliance standards. The MDS Nurse Coordinator (RN) collaborates closely with Nursing, Therapy, Social Work, Dietary, Activities, Business Office, and facility leadership to ensure accurate assessment coding reflective of resident acuity and services provided.

Requirements

  • RN license in the state of New York.
  • Ability to read, write, speak, and understand the English language at an intermediate or more advanced level.

Nice To Haves

  • Prior MDS Coordinator / RNAC experience preferred
  • Long-Term Care and Skilled Nursing experience preferred
  • Experience with PointClickCare (PCC) preferred
  • Knowledge of RAI process, Medicare, OBRA, and reimbursement guidelines
  • Strong organizational and communication skills

Responsibilities

  • Completion and submission of MDS assessments for all payer types and assessment schedules
  • Accurate coding of diagnoses, treatments, medications, functional status, and resident interviews
  • Participation in interdisciplinary care planning and IDT meetings
  • Collaboration with nursing and clinical departments regarding reimbursement and documentation needs
  • Support regulatory compliance and survey readiness efforts
  • Monitor assessment timelines and submission accuracy
  • Communicate clinical reimbursement concerns appropriately
