About The Position

MercyOne Des Moines Medical Center, founded in 1893, is Iowa’s largest medical center with 802 beds and a major referral center, employing over 7,000 colleagues. MercyOne Dyersville Senior Care is a 40-bed facility providing comprehensive services including 24/7 professional nursing care, on-site emergency department, family practice, pharmacy, Medicare Part B rehabilitation, nutritional monitoring, and social work support, all within a personal, home-like setting adjoining MercyOne Dyersville Medical Center. The MDS Nurse Coordinator will be responsible for conducting and coordinating resident assessments in accordance with state requirements and facility policies. This role is a collaborative member/leader of the healthcare team, practicing nursing according to ANA Standards of Practice & Professional Performance, Mercy leadership standards, and external regulatory/accreditation agencies, while actively promoting Mercy Nursing and the nursing profession.

Requirements

• Must possess a current, unencumbered, active license to practice as a Registered Nurse in the state of Iowa.
• Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long-term care.
• Must be AANAC Certified or obtain certification within 2 years of hire.
• Must lead by example and possess superior customer service skills and professionalism.
• Flexibility to adapt to ongoing change and work in a fast-paced, customer driven environment.
• Interpersonal skills to drive collaboration, commitment and productivity when working with cross-functional teams, customers and end users.
• Self-starter, highly motivated with high energy level.
• Demonstrates superior written and verbal communication and presentation skills.
• Basic to intermediate proficiency with Microsoft product suite (MS Word, Excel, Power Point, etc.); basic knowledge and experience with electronic mail and calendaring system in PC LAN environment.
• Ability to type with speed and accuracy.
• Possesses a high degree of personal accountability, responsibility and independent decision-making abilities with the skills to plan, organize, develop, implement and interpret programs, goals, objectives, policies and procedures of the organization.
• Child and Dependent Adult Abuse Mandatory Reporter Training is required within 6 months of hire and every 5 years thereafter.
• Basic Life Support Certification (BLS) will be required within 30 days of hire and every 2 years thereafter.

Nice To Haves

• Bachelor of Science in Nursing (BSN) preferred.
• Previous experience/training in rehabilitative and restorative nursing practices preferred.
• Previous experience working in long-term care or with the geriatric population preferred.

• Practice within the legal scope of a registered professional nurse as outlined in the Iowa Board of Nursing Practice Act and deliver competent care consistent with standards/policies defined by professional nursing organizations, Mercy Medical Center, and external regulators/accreditation/certifying organizations.
• Execute assignments in a culture that is shared and collaborative across all departments.
• Conduct and coordinate the development and completion of the resident assessment (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment, including the implementation of RAPs/CAA and Triggers/CAT’s.
• Conduct or coordinate the interviewing of each Resident for the Resident’s assessment.
• Evaluate each Resident's condition and pertinent medical data to determine any need for special assess activities.
• Assist in the recruitment, interviewing, and selection of personnel for the Department.
• Develop preliminary and comprehensive assessments of the nursing needs of each resident, utilizing the forms required by current rules or regulations and facility policies.
• Coordinate and oversee the entire RAI processes including completion of documentation as necessary to ensure that time frames are met by the interdisciplinary team.
• Coordinate with the interdisciplinary team the development of a written plan of care (preliminary and comprehensive) for each Resident that identifies the problems/needs of the Resident, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
• Ensure that the care plan includes measurable objectives and timetables to meet the Resident's medical, nursing, and mental and psychosocial needs as identified in the Resident's assessment.
• Ensure that an initial resident assessment is completed within appropriate time frame of the resident's admission and locked and electronically submitted timely.
• Ensure that a comprehensive care plan is completed within twenty-one days of the resident's admission and filed in Resident’s chart.
• Ensure that medical and nursing care is administered in accordance with the Resident’s wishes including the implementation of advance directives.
• Assist with monitoring of documentation compliance; identify opportunities with documentation requirements and education on proper documentation.
• Maintain the confidentiality of all resident care information.
• Ensure that all nursing care is provided in privacy.
• Support community personnel from other departments through coaching and mentoring to help achieve optimum standard of excellence.
• Address issues of concern through courageous conversation and notifies department manager of any interactions requiring attention.
• Must function independently, make independent decisions, demonstrate flexibility, personal integrity and ability to work effectively with residents and personnel.
• Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the community policies and procedures.
