About The Position

We are seeking a highly skilled and dedicated MDS LPN or RN to join our team at Country View Nursing. Requirements

• Active LPN or RN license in the state of Missouri
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Excellent organizational and time management skills

Nice To Haves

• Minimum of 2 years of experience in a long-term care setting
• Experience with MDS assessments and care planning
• Experience with electronic medical records systems

Responsibilities

• Conducting patient assessments and developing individualized care plans
• Administering medications and treatments as prescribed by physicians
• Monitoring and documenting patient progress and communicating with healthcare team members
• Providing education and support to patients and their families
• Maintaining accurate and up-to-date patient records
