About The Position

As an MDS & Infection Control Nurse, you are responsible for coordinating and completing the Minimum Data Set (MDS) process while leading the organization's infection prevention and control program. This role partners closely with interdisciplinary teams to ensure accurate clinical documentation, regulatory compliance, quality outcomes, and effective infection prevention practices that support resident well-being.

Requirements

  • Current Registered Nurse (RN) licensure required in State of Minnesota.
  • Knowledge of Medicare, Medicaid, CMS regulations, and reimbursement processes.
  • Strong understanding of infection prevention and control principles, surveillance, and regulatory requirements.
  • Proficiency with electronic health records and clinical documentation systems.
  • Ability to pass state mandated background checks.
  • Physical capability to perform all essential job functions.
  • Ability to read, write, and speak English to ensure effective communication with team members, residents, and community partners.

Nice To Haves

  • Experience with MDS coordination, RAI process, and long-term care regulations preferred.

Responsibilities

  • Prepare, complete, and oversee the MDS process, ensuring assessments, care plans, documentation, and submissions are accurate, timely, and compliant with federal, state, and payer requirements.
  • Partner with interdisciplinary teams, residents, and families to ensure care plans accurately reflect resident needs, validate supporting documentation, monitor quality measures, and determine Medicare eligibility.
  • Develop, implement, and evaluate infection prevention and control policies, conduct infection surveillance, investigate infection trends, and ensure compliance with current infection control regulations and best practices.
  • Train and coach team members on documentation standards, MDS coding, infection prevention practices, regulatory updates, and provide education to residents and families regarding infection control measures.
