About The Position

Visiting Nurse Health System is hiring an MDS Healthcare Coordinator for the Community Care Services Program (CCSP). This office-based clinical position coordinates care for a caseload of patients and helps ensure care meets quality standards and utilization guidelines. The coordinator reviews admission documentation before submitting client assessment data and supplemental medical records through the Georgia Medical Care Foundation (GMCF) online portal, and enters client data into the portal daily. The role provides clinical documentation guidance, education, and training to clinicians and CCSP leadership, including support related to quality issues, denial reasons, and documentation standards. It also communicates with other waiver programs about client status and services, tracks denials and appeals using Excel, and collaborates with the Level of Care Coordinator to obtain signed Levels of Care from primary care physicians before expiration. Although the position is primarily office-based, field visits may be required to determine whether clients meet the level-of-care requirements for CCSP. The schedule is Monday through Friday, 8 a.m. to 5 p.m., at the Hartsfield office in Atlanta, Georgia. The role reviews clinical records, identifies and follows up on needed corrections, supports staff performance and compliance, maintains knowledge of GMCF requirements and Medicare and Medicaid regulations, and contributes to performance improvement and process changes.
