About The Position
Skylake Post Acute is hiring an experienced MDS Coordinator (RN/LPN) to join their team. This is a full-time position at one of the largest skilled nursing facilities (SNF) in Colorado, known for rehabilitation and 24-hour skilled nursing care. The role involves collecting and comparing MDS documents, making necessary corrections, and filing them while maintaining patient confidentiality.
Requirements
- Current, unencumbered, active license to practice as an RN/LPN in Colorado
- Experience as a MDS Coordinator utilizing 3.0
- Experience in a SNF environment
Responsibilities
- Collects MDS documents within mandated, established and expected time frames.
- Compares data entered against the original MDS document; makes corrections as needed.
- Files the MDS documents in residents' charts.
- Maintains confidentiality of all patient care information.
