About The Position

Legend Healthcare and Rehabilitation - Greenville is seeking a full-time MDS Coordinator (RN or LVN) to join their team. The MDS Coordinator is responsible for coordinating the Resident Assessment Instrument (RAI) process, including the completion of an accurate Minimum Data Set (MDS)/PPS and the development of the Interdisciplinary Plan of Care. This role also involves completing LTCMI (Long Term Care Medicare Information) or LOC (Level of Care) forms for the Medicare payment process when applicable, ensuring all duties are performed in a timely manner consistent with CMS guidelines and Legend Healthcare standards. The individual will establish and maintain positive relationships with all members of the health care team.

Requirements

Current RN or LVN license in the state of Texas
Electronic Health Record Experience
Experience with RAI process and MDS certification preferred
Experience in a Skilled Nursing and Long Term Care facility preferred
Experience as a MDS Coordinator preferred
Pledge to live CAPLICO
