About The Position

The Resident Assessment Coordinator (MDS Coordinator) is responsible for maximizing RUGS and CMI scores through the coordination, documentation, and facilitation of all resident assessment activities within the community, adhering to the guidelines of each department. This role ensures the accuracy and completion of ADLs and assists in managing the day-to-day operations of the health and wellbeing component of the community. The goal is to maintain the highest levels of resident functional ability, safety, and wellbeing, in alignment with Juniper Village's philosophy, hallmarks, policies, procedures, and current federal, state, and local regulatory standards.

Requirements

  • An RN or LPN license.
  • A current RN or LPN license in good standing.
  • Previous management experience in healthcare or a service-related setting.
  • Knowledge of all applicable governmental regulations.
  • Knowledge of the physiology and psychology of the older adult.

Responsibilities

  • Ensure compliance with Juniper Village philosophy and hallmarks, as well as all written policies and procedures governing the Community Life and Health and Wellbeing Departments.
  • Ensure resident safety at all times.
  • Embrace Culture Change and resident-centered philosophy.
  • Complete all residents' MDS's and RAPS accurately and timely, including for new admissions, re-admissions, quarterly, yearly, change of conditions, and significant changes.
  • Input all MDS's into the computer.
  • Attend care conferences, audit care conference materials, and sign the MDS.
  • Assure MDS's and care plans are compliant with regulations.
  • Write and update care plans per assessments and monitor that acute care plans are current.
  • Complete all assigned paperwork to meet current regulatory compliance standards.
  • Assist in coordinating effective communication among Resident Associates, residents, and families.
  • Maintain liaison with residents and family members.
  • Assist in the planning and implementation of the In-Touch Orientation and in-service training.
  • Assist in establishing rapport among Wellness team members to facilitate teamwork.
  • Assist in the recruitment and selection of competent Resident Associates.
  • Make written and oral reports to the Director of Wellness or Administrator concerning the operation of the Wellness Department.
  • Work in concert with other members of the Leadership Team.
  • Assist Director of Wellness in various nursing activities.
  • Carry out QAPI roles and responsibilities as assigned.
  • Follow established policies and procedures in support of QAPI efforts.
  • Assist in the coordination of regularly scheduled wellness meetings.
  • Complete all administrative paperwork as required.
  • Work in conjunction with resident’s attending Physician, as needed, to ensure the highest quality of service is being provided.
  • Submit recommendations to the Director of Wellness or Administrator for budget completion, equipment, and supplies.
  • Maintain confidentiality of all resident and staff information.
  • Serve as a networking team member, as directed.
