About The Position

The role of the MDS Coordinator and associated responsibilities influence the organization’s case mix reimbursement, Quality Indicator profiles, and the state surveyor focus. The MDS Coordinator is responsible for the timely and accurate completion of both the RAI and Care Management process from admission to discharge in accordance with Company Policy and Procedures, MPGs, State and Federal guidelines, and all other entities as appropriate.

Requirements

  • High school diploma or equivalent experience
  • Graduate of Accredited School of Nursing with current LPN or RN Illinois license
  • 3 - 5 years or more of MDS experience
  • Proficiency with current version of MDS software
  • 5 years or more of Skilled Nursing experience
  • Strong assessment and analytical skills, a thorough understanding of MDS requirements

Responsibilities

  • Must have a thorough knowledge and understanding of MDS (current version) and RAI rules and regulations
  • Work in collaboration with the Interdisciplinary Team to assess the needs of the resident
  • Determine admission diagnosis with corresponding codes
  • Assist with coordination and management of the weekly Medicare meeting to include review of resident care and the setting of the Assessment Reference Date
  • Comply with Federal and State regulations regarding completion and coordination of the RAI process
  • Monitor MDS and care plan documentation for all residents - Ensure documentation is present in the medical record to support MDS coding
  • Develop individualized care plans for the residents and update resident care plans when there’s a change in condition and as required
  • Ensure all departments complete their portions of the MDS in a timely manner
  • Ensure that therapy documentation and nursing documentation are appropriate and in place
  • Maintain current MDS status of assigned residents according to State and Federal guidelines
  • Support the tracking system of MDS schedules (timeframes and due dates)
  • Maintain the frequent and accurate data entry of resident information into appropriate computerized MDS programs
  • Maintain Medicare census log including admissions, discharges, and number of Medicare days billable, and percent of Medicare days denied
  • Complete accurate coding of the MDS with information obtained via medical record review as well as through observation and interview with facility staff, resident, and family members
  • Attend interdisciplinary team meetings and other meetings in order to gather information, communicate changes, and maintain and update records
  • Continually updating knowledge base related to data entry and computer technology
  • At the direction of the Administrator/DON, may participate in quality assurance activities
  • Contact Medicare Beneficiary Hotline per company policy and Administrator/DON direction
  • Complete electronic submission of the MDS and required documentation to the State database and other entities per company policy and notify the Billing department so billing can be done
  • Schedule care plan meetings with resident and families twice a week
  • Oversee the Restorative program and the Psychotropic Reduction program
  • Ensure that Doctor’s Certification Sheet is signed by the Medical Director or Attending Physician
  • Serve as Restorative Nurse
  • Chair the weekly Medicare meeting
  • Responsible for notification of patient, family/responsible party regarding termination of benefits. Completes all documentation regarding decertification.
  • Weekly updates of Medicare Profile and Nursing Guidelines.
  • On an on-going basis, add to plan as appropriate.
  • Oversee therapies and rehab documentation to make sure appropriate documentation is in place.
  • On a continuing basis, instruct nurses on correct and appropriate charting. If nursing is primary basis for Medicare charting, documentation must support that.
  • On a continuing basis, instruct and monitor CNA documentation (POC) to adequately reflect the assistance and care provided to the residents by the staff.
  • Maintain Medicare census log including admissions, discharges, and number of Medicare day’s billable, percent of Medicare days denied.
  • Continually audit Medicare charts for documentation compliances: Follow-Up Doctor’s care plan, certification sheet signatures, face sheet, level of care, etc.
  • Adhere to the policies and procedures of the business with the highest practical standards
  • Participate in and maintain current status on all training and education required by the company
  • Communicate effectively with all members of the staff, residents and business partners
  • Provide feedback to others for continuous quality improvement of the entire organization
  • Openly accept feedback from others for the continuous quality improvement of the entire organization
  • Escalate issues when there is appearance that business standards of quality are not present
  • Comply with all rules and regulations of the organization, including HIPAA and safety
  • Perform other duties as assigned
