Requirements
- LPN or RN license
- Strong, detail-oriented skills
- Knowledge of MDS assessments, care planning, and reimbursement processes
- Understanding of Medicare, Medicaid, and state regulations
- Proficiency in EHR systems
Responsibilities
- Complete and manage accurate, timely MDS assessments in accordance with federal and state guidelines
- Collaborate with the interdisciplinary team to develop and update individualized care plans
- Ensure compliance with Medicare/Medicaid regulations and documentation standards
- Utilize EHR systems to maintain precise clinical documentation
- Support reimbursement optimization and clinical documentation improvement
