About The Position

Acts is seeking a qualified MDS/Care Coordinator for its skilled nursing neighborhood. This role involves coordinating care for existing residents and new admissions, including initial and routine assessments, and collaborating with interdisciplinary team members. The coordinator ensures compliance and accurate completion of the Minimum Data Set (MDS) and oversees the care planning process. They act as a liaison between the community, payer, and corporate office, and coordinate the restorative nursing care program by screening, setting goals, and educating staff. Requirements

  • Current State Registered Nurse (RN) license.
  • Minimum of two years' experience with Minimum Data Set (MDS) in a long-term care setting.
  • Current or eligible for certification in CPR.

Responsibilities

  • Coordinate care for existing residents and new admissions to the skilled care community (SCC).
  • Conduct initial and routine assessments and coordinate assessments by other interdisciplinary team members.
  • Ensure compliance and accurate completion of the Minimum Data Set (MDS).
  • Oversee and develop the care planning process.
  • Serve as the liaison between the community, the payer, and the corporate office.
  • Coordinate the restorative nursing care program by screening, setting and updating goals for residents, and educating staff on the implementation.
