About The Position

The RN MDS Coordinator coordinates and assists with completing and submitting accurate, timely interdisciplinary MDS Assessments, CAAs, and Care Plans in accordance with CMS RAI Manual regulations, applicable laws and regulations, and Life Care standards. The role also supports documentation quality, provides guidance to direct care associates on regulatory updates, and participates in interdisciplinary and facility meetings. The coordinator may perform staff nurse functions as required.

Requirements

Associate’s or bachelor’s degree in nursing from an accredited college or university.
Current license or registration in the applicable state; must maintain an active Registered Nurse (RN) license in good standing throughout employment.
Two years of nursing experience.
CPR certification upon hire or obtained during orientation, and kept current throughout employment.
Advanced knowledge in the field of practice.
Ability to make independent decisions when circumstances warrant.
Knowledge of practices, procedures, laws, regulations, and guidelines governing post-acute care facility functions.
Ability to implement and interpret departmental programs, goals, objectives, policies, and procedures.
Proficiency in competency areas including patient rights, safety, and sanitation.
Ability to maintain professional working relationships and confidentiality of proprietary or confidential information.
Ability to follow company policies, including harassment and compliance procedures, and adhere to Life Care’s Code of Conduct and required compliance training.
Ability to concentrate, use reasoning skills and good judgment, and read, write, speak, and understand English.
Ability to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours.

Responsibilities

Coordinate and assist with completion and submission of accurate, timely interdisciplinary MDS Assessments, CCAs, and Care Plans in accordance with CMS RAI Manual regulations.
Report changes in a patient’s condition identified through MDS Assessments to the DON.
Educate direct care associates about CMS RAI Manual updates or changes to Skilled Nursing Facility regulations that affect documentation.
Assist with review of the Interdisciplinary Comprehensive Care Plan.
Review Final Validation Reports and attest that assessments are completed and accepted into the CMS QIES system before billing; notify the Business Office when assessments are not ready to bill.
Review CMS Reports to identify late assessments and develop systems and processes to prevent recurrence.
Attend and participate in the Daily PPS Meeting, Monthly Triple Check, and other meetings upon request.
Perform staff nurse functions as required.
Provide excellent customer service and maintain a positive attitude toward patients.
Assist with patient evacuation.
Maintain dependable, regular attendance.
Communicate and function productively on an interdisciplinary team.
