About The Position

Kāhala Nui , considered Hawaiʻi’s premier senior living community, is seeking a dedicated and detail-oriented MDS Coordinator to join our team. POSITION SUMMARY: Provides oversight of the assessment and care planning process in the nursing department. The MDS Coordinator is responsible for ensuring that MDS (Minimum Date Set) is accurate, complete, follows the CMS MDS RAI guidelines (for Medicare and Medicaid) and is submitted to the appropriate agency or provider in a timely manner. Coordination of resident admissions and initial services with members of the Hi’olani Interdisciplinary team (IDT). May provide general nursing care to residents in the Comprehensive Nursing units at Hi’olani Care Center on an as needed basis as directed by the Director or Assistant Director of Nursing Services. THIS POSITION IS 100% ON SITE, NO REMOTE OR HYBRID WORK AVAIALABLE. SCHEDULE TO BE DETERMINED BY MANAGEMENT AND DEPARTMENT NEEDS.

Requirements

  • Graduate of an accredited school of nursing.
  • current, valid resisted Nurse (RN) license in good standing in the State of Hawaii.
  • Working knowledge of the Minimum Data Set (MDS and the Resident Assessment Instrument (RAI.
  • Ability to read, write, and understand to carry out instructions furnished in written, oral, or diagram form.
  • Ability to solve problems under minimal supervision.
  • Proficiency with basic computer functions, including email, web browsers, standard office application, and electronic health records system to document resident care and clinical workflows.
  • Proof of annual tuberculosis clearance in accordance with state regulations.
  • Valid CPR, First Aid and AED certification.
  • Must be available to work varied schedules including weekends and holidays.

Nice To Haves

  • Previous experience working as an MDS Coordinator in a long-term care or skilled nursing facility.
  • Minimum one (1) year of nursing experience in a long-term care or skilled nursing facility.
  • Active RAC-CT certification through the American Association of Post-Acute Care Nursing (AAPACN).
  • Knowledge of Resident Assessment Instrument (RAI) guidelines and Medicare/Medicare reimbursement processes.
  • Experience with care planning and quality improvement initiatives in skilled nursing facilities.
