MDS Coordinator
About The Position
Clement Manor is seeking a full-time MDS Coordinator for its faith-based independent living, assisted living, CBRF, and long-term care community. The MDS Coordinator oversees and coordinates the development and completion of resident assessments (MDS), including the CAA’s, in accordance with current federal and state rules, regulations, and guidelines governing resident assessment. The position participates in an on-call rotation every five weeks and is included in the on-call holiday rotation. Requirements
- Graduate of an accredited School of Nursing.
- Current Wisconsin state nursing licensure.
- Understanding of and ability to comply with federal and state rules and regulations applicable to nursing homes, such as PDPM.
- Strong assessment skills.
- Ability to facilitate and communicate with all disciplines.
- Ability to organize and prioritize.
- Knowledge of computer applications and processes.
Nice To Haves
- Current AANAC certification is preferred but not required.
