Requirements
- Licensed Practical Nurse or Registered Nurse in the State of South Carolina
- Two years of clinical experience in the healthcare setting
- One year of experience in a long-term care setting
- One year of experience as MDS Coordinator
- Strong clinical assessment skills
- Intermediate computer skills
- CPR certified
- Valid SC Driver’s License
Responsibilities
- Lead and oversee clinical care programs in compliance with organizational standards, SC Nurse Practice Act, and federal certification guidelines.
- Demonstrate knowledge of the survey process, educate and assist the staff in becoming aware and comfortable with the process.
- Coordinate and oversee individualized care plans, ensuring nursing care aligns with resident wishes, advance directives, and all regulatory requirements.
- Drive quality improvement through planning, implementing, and evaluating QA initiatives.
- Support admissions by assessing resident needs to ensure appropriate community care placement.
- Coordinate the interdisciplinary team in implementing the RAI process in accordance with current federal and state rules, regulations and guidelines.
- Ensure accurate completion of assessments and care plans through interdisciplinary review and communication with staff, physicians, and families.
- Support Medicare coverage decisions, set ARDs, and ensure compliance with Medicare requirements.
- Disseminate any new or updated materials involving the RAI process to the interdisciplinary team.
- Ensure timely electronic submission and general validity of all MDS’s to the federal and state database.
