About The Position

Grand Meadows Senior Living and Health Care is seeking a MDS Coordinator | RN to join their team. This role is responsible for the timely and accurate completion of the RAI process and care management process from admission to discharge, in accordance with company policy, Federal, State, and Certification guidelines. The position may also involve coordinating information systems operations and education for the clinical department, under the direction of the Director of Nursing and VP of Clinical Reimbursement.

Requirements

• Currently licensed as a Registered Nurse (RN) in the state in which you are practicing.
• Professional image in both appearance and behavior.
• Excellent written and oral communication skills.
• Knowledge of the care planning process required.
• Thorough understanding of the Quality Indicator process, OBRA regulations, and Minimum Data Set.

• Flexible, fun, and energetic personality.
• Prior MDS experience highly preferred.
• Minimum of one year of experience in a LTC/SNF/AL/MC setting highly preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement highly preferred.

Responsibilities
