In this exciting role, you’ll do more than just complete MDS paperwork – you’ll be a key resource, a team leader, and an advocate for excellence in resident care.
Requirements
- A licensed Registered Nurse (RN) with a strong clinical background.
- Experience with MDS, care planning, and Medicare coordination is required.
- A collaborative spirit and leadership mindset.
- Someone who thrives in a fast-paced, team-oriented environment and is passionate about senior care.
Responsibilities
- Coordinate and manage the Medicare program for the Health Center.
- Complete accurate and timely Minimum Data Set (MDS) documentation.
- Serve as a knowledgeable resource to unit staff, promoting best practices and collaboration.
- Step in as a staff nurse when needed to ensure seamless care delivery.
- Assist in oversight of day-to-day operations and ensure high-quality service for residents in Creekview Rehab.
- Participate in the on-call rotation, offering support, consultation, and leadership coverage.
- Be an active member of the Creekview Leadership Team, contributing to strategic planning and operational excellence.
