About The Position

The Minimum Data Set Coordinator is responsible for the accurate and timely completion of the Minimum Data Set (MDS), Care Area Assessments (CAAs), and Resident Assessment Instrument (RAI) processes in accordance with Centers of Medicare/Medicaid (CMS) regulations, New Jersey Department of Health (NJDOH) requirements, PDPM (Patient-Driven Payment Model) and Voorhees Pediatric Facility (VPF) policies. This role ensures precise assessment, documentation, and interdisciplinary communication to support high ‑ quality resident care, accurate reimbursement, and regulatory compliance.

Requirements

  • Current New Jersey Registered Nurse license in good standing.
  • 1–2 years of experience in long-term care or subacute rehabilitation.
  • Proficiency in electronic health records and MDS software systems.
  • Strong knowledge of RAI Manual, PDPM, and NJDOH Long Term Care regulations.
  • Exceptional attention to detail.
  • Strong analytical and critical-thinking skills.
  • Ability to manage deadlines and multiple assessments simultaneously.
  • Strong understanding of clinical documentation and regulatory standards.
  • Effective communication and collaboration with interdisciplinary teams.

Nice To Haves

  • Certification as a Resident Assessment Coordinator (RAC ‑ CT).
  • Experience with NJ Medicaid Case Mix Index (CMI) system.
  • Prior leadership or staff education experience.

Responsibilities

  • Complete and submit MDS assessments accurately and within required federal and state timelines.
  • Coordinate and lead the Resident Assessment Instrument (RAI) process.
  • Maintain compliance with CMS, NJDOH regulations, and facility policies.
  • Initiate and complete Care Area Assessments (CAA)
  • Ensure timely completion of Significant Change in Status Assessments (SCSA)
  • Analyze MDS Data to identify opportunities for improvement
  • Contribute to Quality Assurance and Performance Improvement (QAPI) initiatives.
  • Develop and revise individualized care plans based on MDS findings
  • Ensure assessments support accurate Patient-Driven Payment Model (PDPM)
  • Monitor the Case-Mix Index (CMI) for NJ Medicaid
  • Serve as the primary inter-departmental liaison, educator and supervisor of MDS related care.
  • Preparation and participation in NJDOH surveys, audits, and quality reviews.
