About The Position

Asbury Heights, a premier senior living community managed by Oakdale Seniors Alliance, is seeking a high-energy, meticulous Part-Time MDS Coordinator (LPNAC) to join their team. This role offers a rewarding, high-impact position on their Pittsburgh campus with the flexibility needed outside of work. The MDS Coordinator will master the MDS process by coordinating and overseeing the timely completion of all MDS assessments (OBRA and PPS) from admission to discharge, ensuring 100% accuracy and compliance with Pennsylvania state and federal guidelines. They will collaborate with the Interdisciplinary Team (IDT) including nursing, therapy, dietary, and social services to build truly person-centered care plans. The role involves maximizing clinical reimbursement by reviewing clinical documentation to support care provided and capture appropriate resource utilization data. Additionally, the MDS Coordinator will conduct routine quality reviews on medical records, provide feedback to floor staff on documentation, and assist in ensuring the smooth, error-free electronic submission of all MDS data to CMS.

Requirements

  • An active, unencumbered Licensed Practical Nurse (LPN) license in the state of Pennsylvania.
  • Hands-on experience managing or assisting with the MDS / RAI process in a skilled nursing or long-term care setting.
  • Meticulous Attention to Detail: An analytical mindset with a passion for compliance, data integrity, and complex problem-solving.
  • Great Communication: Exceptional organizational skills and the ability to collaborate smoothly across different departments.

Nice To Haves

  • RAC-CT (Resident Assessment Coordinator Certified) designation or LPNAC equivalent is a massive plus, but a proven track record of clinical coding excellence is highly valued!

Responsibilities

  • Coordinate and oversee the timely completion of all MDS assessments (OBRA and PPS) from admission to discharge, ensuring 100% accuracy and compliance with Pennsylvania state and federal guidelines.
  • Work closely with nursing, therapy, dietary, and social services to build truly person-centered care plans across our campus that accurately reflect resident needs.
  • Review clinical documentation with a sharp eye to ensure it fully supports the care provided, capturing the appropriate resource utilization data for accurate reimbursement.
  • Conduct routine quality reviews on medical records and provide helpful feedback to floor staff regarding the vital importance of complete, precise clinical documentation.
  • Assist in ensuring the smooth, error-free electronic submission of all MDS data to CMS, executing course corrections whenever needed.
