The MDS Coordinator (LPN) is responsible for ensuring the accurate completion and submission of all sections and disciplines of the MDS. This role ensures regulatory compliance set by CMS and RAI Manual by staying up-to-date on the latest regulations, conducting audits, and implementing corrective measures needed to ensure maximum reimbursement through CMS. The position also involves implementing and updating other required assessments, initiating significant changes and corrections, and educating staff on discrepancies in ADL coding or documentation. The MDS Coordinator ensures resident care plans are initiated, up-to-date, resident-centered, and that correct interventions are developed, implemented, and evaluated for effectiveness. This role conducts comprehensive assessments and evaluations, documents findings, and ensures regulation compliance. The MDS Coordinator completes and signs necessary documents, coordinates MDS-related IDT meetings, and leads weekly care plan meetings. They attend scheduled MDS/IDT/Care Plan meetings and ensure daily and weekly documentation requirements for skilled/med part B/LTC residents are obtained and correct. This position serves as a resource person to all disciplines regarding the RAI process, care planning, and resident assessments, and initiates MDS-related staff in-services as needed. The role may perform other duties as assigned, assist in emergency declarations, work the floor/cart during critical staffing needs, and serve in the Nurse on-call rotation.
