Under the guidance of the Director of Nursing, the incumbent of this position will oversee the Minimum Data Set (MDS), supervises nursing team members to provide the professional nursing care for the residents of the long-term care facility in accordance with the guidelines set down by the company and the government entities who oversee regulation of long-term care facilities. This position is an active member of the Nursing Leadership Team, with a focus on staff documentation and resident care.

Requirements

  • Active LPN license in the state of practice
  • Two or more years of related supervisory experience in a geriatric environment.
  • Must be CPR certified upon hire or within 31 days of beginning employment with EverTrue
  • Demonstrates the initiative and the ability to work independently and prioritize tasks
  • Must be able to follow direction written or oral
  • Must have patience, tact, flexibility, a cheerful disposition, enthusiasm as well as the willingness to handle difficult situations
  • Must have legible handwriting, good telephone and filing skills

Responsibilities

  • Coordinates and completes the Minimum Data Set (MDS) with the Interdisciplinary Team (IDT); Obtains sign-off from the Director of Nursing (or their designee) who is a registered nurse (RN)
  • Coordinates and monitors the documentation of care delivery
  • Maintains consistent application of EverTrue policies; participates in coaching/counseling of nursing team members as necessary
  • Performs administrative duties, including, but not limited to, workforce performance evaluations, determining work assignments and scheduling, medical record documentation, completion of RAI/MDS, and other written and oral reports as required
  • Participates in direct resident care when necessary
  • Participates in Committee meeting as required
  • Assists with in-service training of the nursing team members as required
