The MDS Coordinator (LPN) is responsible for ensuring the accurate completion and submission of all sections and disciplines of the MDS (Minimum Data Set). This role ensures regulatory compliance set by CMS and the RAI Manual by staying up-to-date on the latest regulations, conducting audits, and implementing corrective measures to ensure maximum reimbursement through CMS. The position also involves implementing and updating other required assessments, initiating significant changes and corrections, and educating staff on discrepancies in ADL coding or documentation. The MDS Coordinator ensures resident care plans are initiated, up-to-date, resident-centered, and that correct interventions are developed, implemented, and evaluated for effectiveness. This includes conducting comprehensive assessments, documenting findings, and ensuring regulation compliance. The role also involves signing necessary documents, coordinating MDS-related IDT meetings, and leading weekly care plan meetings. Additionally, the MDS Coordinator serves as a resource person for the RAI process, care planning, and resident assessments, and may perform other assigned duties, assist in emergencies, work the floor/cart during critical staffing needs, and serve in the Nurse on-call rotation. The position supports the mission of PVCC by fostering positive working relationships and adhering to the Code of Conduct.
