About The Position

Sun River Health is seeking a full-time MAT Peer Adherence Worker to join our Coram location. This role provides recovery support to reduce barriers of care and shares strategies to improve patient health outcomes. Responsibilities include educating patients regarding MAT, HCV, risk reduction, medical and medication adherence, and providing education resources and materials that serve to promote recovery support education on harm reduction, MAT, Naloxone, and treatment. The role also involves providing Harm reduction and opioid overdose and prevention training and support.

Requirements

Minimum Education: High School Diploma/GED
Minimum Work-Related Experience: 1 year

Nice To Haves

Preferred Education: Associate’s degree, Certified Recovery Peer Advocate (CRPA)

Responsibilities

Uses Harm reduction strategies to engage new patients, provide recovery support, and support in the case of overdose.
Has knowledge of community resources and links patients to recovery supports and community services.
Understanding of and compliance with all HIPPA, OASAS, MAT, and other health-related legal policies and procedures, as well as internal Sun River Health Center policies and procedures.
Demonstrates an understanding of harm reduction principles and has respect for everyone’s unique path to recovery.
Facilitates Naloxone, Fentanyl test strip, Xylazine test strip, and harm reduction training for staff, patients, and the community.
Assists patient with completing recovery support plans.
Participate in clinical meetings, quality improvement, case conferences, staff meetings, and supervision.
Ability to complete electronic and written forms and reports in an accurate and timely manner.
Participates in ongoing training, supervision, and professional development opportunities.
Provides health education and adherence support to patients.
