The social worker is responsible for the implementation of standards of care for medical social services and understanding the community development principles and practices, with a focus on building and sustaining community partnerships. The social worker assesses the psychosocial status of patients related to the patient’s illness and environment and communicates findings to the registered nurse. They carry out social evaluations and plan interventions based on evaluation findings, and maintain clinical records on all patients referred to social work. This role provides information and referral services for patients and families/caregivers regarding practical and environmental needs, and serves as a liaison between patients or families/caregivers and community agencies. The social worker maintains collaborative relationships with company personnel to support patient care, and works in partnership with the sales team to develop strategies to increase community involvement and engagement, promote services, and represent the organization at community events. They also maintain and develop contracts with public and private agencies as resources, participate in the development of the total plan of care and case conferences, and assist physicians and other team members in understanding significant social and emotional factors related to health problems. Participation in discharge planning is also a key responsibility.
