About The Position

Provides psychosocial services to patients treated by the facility, including in-center and home dialysis patients. Works with the health care team to promote positive adjustment, rehabilitation, and improved quality of life. Collaborates with the interdisciplinary team to educate and support staff in understanding the emotional, psychological, and behavioral impact of Chronic Kidney Disease on patients and families. Supports the company's commitment to Quality Indicators, Outcomes, and Quality Assessment and Improvement (QAI) Activities, actively participating in process improvement to enhance patient satisfaction and quality of life. This is an entry-level MSW role.

Requirements

  • Masters in Social Work
  • State temporary license, Registered Social Worker Clinical Intern, Associated Social Work license or Provisional LCSW License (if required by state)
  • Obtains necessary state licensure to work independently without supervision within the first two (2) years or as required by state guidelines.
  • 0 – 2 years’ related experience

Responsibilities

  • Assesses patients' psychosocial status, strengths, and needs that may affect rehabilitation and treatment outcomes.
  • Participates in care planning with the patient and healthcare team to identify interventions for rehabilitation, treatment goals, and quality of life.
  • Utilizes patient education programs, social work theory, and quality of life measurement instruments for assessment and care planning.
  • Provides monitoring and interventions for patients to adjust to dialysis and achieve optimal psychosocial status and quality of life.
  • Provides supportive counseling services within the scope of clinical training and state license.
  • Offers educational and goal-directed counseling to patients seeking transplant.
  • Provides information and assists with referrals to community resources (e.g., home health services, vocational rehabilitation).
  • Maintains current knowledge of local vocational/educational rehabilitation programs and assists patients with referral and access.
  • Assesses patient awareness of advance directives, assists with accessing forms/information, and facilitates discussions.
  • Participates in discussions regarding patient DNR status with the physician and nurse.
  • Adheres to company Social Work Policy and Measuring Patient Physical and Mental Function Policy, including documentation.
  • Documents MSW interactions and interventions provided to patients and/or families.
  • Provides psychosocial support and/or Social Work Focused Interventions for non-adherence, quality outcomes, and quality of life concerns.
  • Participates in monthly Quality review meetings with the interdisciplinary team, reporting on quality indicators.
  • Assesses patient knowledge of kidney disease and barriers to treatment adherence, providing tailored education.
  • Provides information about all treatment modalities with the interdisciplinary team.
  • Facilitates the transplant referral process and collaborates on transplant waitlist management.
  • Provides ongoing education to patients/families regarding psychosocial issues related to End Stage Renal Disease (ESRD) and available support services.
  • Reviews patient rights and responsibilities, grievance information, and facility policies with patients/representatives.
  • Collaborates with the team on appropriate QAI activities.
  • Reviews Patient Rights and Responsibilities, Grievance Procedure, FKC Non-discrimination policy, and DNR Statement upon admission.
  • Understands the referral and admission process and supports the clinic in patient scheduling for adherence and adjustment.
  • Interviews patients to identify causes for discharge requests and shares concerns with operational leadership.
  • Collaborates with Insurance Coordinators to address insurance-related issues.
  • Provides information and education to patients about payment options for dialysis.
  • Collaborates with the Insurance Coordinator regarding changes in patient state that impact insurance.
  • Refers patients to the patient billing solutions (PBS) department for treatment-related bill inquiries.
  • Assists with the interview process and hiring decisions for new personnel if requested.
  • Works with administrative support staff to maintain updated patient resource lists.
  • Provides training to staff on psychosocial topics as needed.
  • Contributes and participates in weekly team huddles, discussing urgent patient issues.
  • Adheres to work-defined caseload guidelines based on state regulatory requirements.
  • Works under appropriate supervision to meet state requirements in applicable states.
  • Performs other related duties as assigned.
