About The Position

Provides psychosocial services to patients treated by the facility, including in-center and home dialysis patients. Works with the health care team to promote positive adjustment, rehabilitation, and improved quality of life. Collaborates with the interdisciplinary team to educate and support staff in understanding the impact of Chronic Kidney Disease on patients and families. Supports Fresenius Kidney Care (FKC) quality initiatives and actively participates in process improvement activities. This is an entry-level MSW role.

Requirements

  • Masters in Social Work
  • Must have state required license
  • Meets the applicable scope of practice board and licensure requirements in effect in the State in which they are employed
  • 0 – 2 years’ related experience

Responsibilities

  • Assesses patients' psychosocial status, strengths, and needs affecting rehabilitation and treatment outcomes.
  • Participates in care planning with the patient and healthcare team to identify interventions for rehabilitation, treatment goals, and quality of life.
  • Utilizes FKC patient education programs, social work theory, and quality of life instruments for assessment and care planning.
  • Provides monitoring and interventions for patient adjustment to dialysis and psychosocial status.
  • Offers supportive counseling services within the scope of clinical training and state license.
  • Provides educational and goal-directed counseling for patients seeking transplant.
  • Refers patients to community resources (home health, vocational rehabilitation) to facilitate optimal treatment outcomes.
  • Maintains knowledge of local vocational/educational rehabilitation programs and assists patients with referrals.
  • Assesses patient awareness of advance directives, assists with forms, and facilitates discussions.
  • Participates in discussions regarding patient DNR status with the physician and nurse.
  • Documents social work interactions and interventions.
  • Provides psychosocial support and interventions for non-adherence, quality outcomes, and quality of life concerns.
  • Participates in monthly Quality review meetings and reports on quality indicators.
  • Assesses patient knowledge of kidney disease and barriers to treatment adherence.
  • Educates patients and families about kidney disease and treatment modalities.
  • Facilitates the transplant referral process and collaborates on waitlist management.
  • Provides ongoing education on psychosocial issues related to ESRD and available support services.
  • Reviews patient rights and responsibilities, grievance information, and facility policies.
  • Collaborates with the team on QAI activities.
  • Reviews patient rights and responsibilities, grievance procedures, and other policies during admission.
  • Interviews patients to identify reasons for discharge requests.
  • Liaises with Insurance Coordinators to address insurance-related issues.
  • Educates patients about payment options for dialysis (federal, state, commercial insurance, etc.).
  • Collaborates with the Insurance Coordinator on changes affecting patient insurance.
  • Refers patients to the patient billing solutions (PBS) department.
  • Assists with the interview process for new personnel if requested.
  • Works with administrative staff to maintain updated patient resource lists.
  • Provides training to staff on psychosocial topics.
  • Participates in weekly team huddles and discusses urgent patient issues.
  • Adheres to work defined caseload guidelines.
  • Performs other related duties as assigned.
