Requirements

• Masters Level Social Worker.
• Master's degree in Social Work required.

Nice To Haves

• Drivers License.
• 3 years of experience preferred.

Responsibilities

• Interview participants to evaluate social, emotional, economic, and environmental needs, prioritizing issues to develop and document comprehensive intervention and post-discharge plans.
• Consult with internal management, statutory/voluntary agencies, and caregivers to ensure treatment compliance while connecting participants to community health, welfare, and social resources.
• Support concurrent reviews, discharge planning, home care, and ongoing social service coordination by building strong networks and identifying creative solutions for participant needs.
