About The Position
We are seeking a compassionate and experienced Master of Social Work (MSW) professional for a travel assignment in Napa, California. The ideal candidate will provide psychosocial support, discharge planning, care coordination, and advocacy services while collaborating closely with interdisciplinary healthcare teams.
Requirements
- Master’s Degree in Social Work (MSW) from an accredited program required
- Current California Social Work licensure or eligibility required
- Previous experience in a healthcare, hospital, or acute care setting preferred
- Strong knowledge of discharge planning, care coordination, and community resource management
- Excellent communication, interpersonal, and counseling skills
- Ability to work effectively in a multidisciplinary healthcare environment
- Strong assessment, critical thinking, and problem-solving abilities
- Experience in acute care, medical social work, or case management preferred
- Familiarity with EMR systems preferred
- Knowledge of Medicare, Medicaid, and healthcare regulations preferred
- Experience working with diverse patient populations preferred
Responsibilities
- Conduct comprehensive psychosocial assessments of patients and families
- Develop and implement individualized care and discharge plans
- Provide crisis intervention, counseling, and emotional support services
- Assist patients with accessing community resources, financial assistance programs, and support services
- Collaborate with physicians, nurses, case managers, and healthcare professionals to coordinate patient care
- Advocate for patients’ rights and promote patient-centered care
- Facilitate discharge planning to ensure safe and effective transitions of care
- Identify barriers to treatment and develop strategies to address patient needs
- Maintain accurate and timely documentation in accordance with facility and regulatory standards
- Participate in multidisciplinary team meetings and case reviews
- Educate patients and families regarding available healthcare and community resources
- Ensure compliance with hospital policies, state regulations, and ethical standards of social work practice
