The Baltimore City Health Department is seeking a committed individual to serve as a Maryland Access Point (MAP) Community Health Worker within the Division of Aging and CARE Services. Incumbents receive close supervision from a team leader/supervisor. Work involves responding to inquiries by phone and in person regarding services for older adults and persons with disabilities, conducting community outreach, and connecting constituents to services and resources. A Grant Services Specialist II provides a wide variety of grant-funded paraprofessional administrative, programmatic and complex general support services work to an assigned City agency or program. Work of this class involves no supervisory duties or responsibilities. Employees in this class work a conventional workweek that may include evening and weekend hours. Work is generally performed in an office or clinic where working conditions are normal. Work generally requires minimal physical exertion.
