About The Position

US Heart and Vascular is needing a Remote Value Based Care Programs Manager to join our team. Position Summary: The Manager, Value Based Care (VBC) Programs drives provider, practice, and patient engagement in VBC programs across US Heart and Vascular (USHV). This role leads the development and distribution of performance dashboards, delivers ongoing physician and staff education, supports Joint Operating Committee (JOC) governance, and serves as a subject matter expert (SME) for VBC programs. The manager creates program collateral and communications, promotes patient engagement strategies, and partners cross-functionally to achieve performance to maximize VBC incentive opportunities.

Requirements

  • Expertise in data visualization, predictive modeling, and performance measurement
  • Exceptional communication and presentation skills, with ability to influence executive stakeholders
  • Proven history of collaborating with payers, providers, or both on high-priority, high-visibility initiatives
  • Strong proficiency in data analysis tools for healthcare analytics and experience with reporting platforms such as Tableau.
  • Excellent communication skills with the ability to translate complex data insights into actionable recommendations for stakeholders
  • Knowledge of principles and practices of management and supervision.
  • Understanding financial documents, budgets and resources allocation practices.
  • Strong attention to detail and analytical skills, with the ability to accurately capture and interpret diagnostic images.
  • Effective communication and teamwork skills, with the ability to work collaboratively with healthcare professionals and patients.
  • Ability to handle potentially stressful situations with professionalism and composure.
  • Knowledge of federal, state, and local laws, statutes, regulations, codes, and standards related to the area of responsibility.
  • Knowledge of the principles, concepts, and theories relevant to the assigned functional area.
  • Skill in completing assignments accurately and with attention to detail.
  • Ability to communicate clearly and concisely both orally and in writing in English, to various audiences.

  • Working knowledge of Office 365 programs such as Excel, Outlook, Word, PowerPoint, etc.
  • Supports the Mission, Vision and Values of USHV and the practices.
  • Bachelor’s degree in healthcare administration, Public Health, Nursing, Business, or related field (or equivalent experience).
  • Three (3) years' experience in value-based care, quality programs, population health, practice transformation, or payer/provider performance management.
  • Demonstrated ability to drive provider engagement and performance improvement across multiple sites/practices.
  • Strong analytics fluency and experience building/using performance dashboards and scorecards.
  • Proven facilitation and education experience with physicians and multidisciplinary teams.
  • Excellent communication and stakeholder management skills; ability to influence without direct authority.

Responsibilities

  • Program Engagement & Adoption Owns and executes the engagement strategy for participating practices/providers across all VBC programs.
  • Establishes routine touchpoints with practice leaders and clinical champions to drive participation, performance accountability, and consistent execution of program workflows.
  • Identifies barriers to adoption (workflow, staffing, data, operational constraints) and coordinates solutions with operations, IT, quality, and clinical leadership.
  • Builds and maintains an engagement cadence (newsletters, office hours, learning sessions, campaign calendars) to sustain performance momentum.
  • Develops and delivers training and educational materials to physicians and staff.
  • Performance Dashboards, Reporting & Insights Creates, maintains, and distributes provider/practice performance dashboards that track, utilization and cost trends, patient engagement measures, physician engagement. measures, and contract-specific targets and incentive status
  • Translates data into actionable insights, highlighting priorities, root causes, and recommended interventions.
  • Provides physician and practice-level scorecards and targeted “next best actions” to support measurable improvement.
  • Partners with analytics and data teams to validate data sources, definitions, refresh schedules, and reporting accuracy.
  • Independently analyzes and interprets healthcare utilization and financial data to address business questions related to population health, health outcomes, and contract design.
  • JOC and Communications Support Supports JOCs by preparing performance decks and dashboards.
  • Tracks action items, owners, timelines, and outcomes that support JOC and programs.
  • Creates physician, practice, and patient facing collateral that supports engagement and adoption.
  • Works with market operations and VBC operations to identify high impact outreach opportunities.
  • Uses analytical skills to identify variances in expected clinical program performance, and demonstrates problem-solving skills and business knowledge to make recommendations for process remediation or improvement.
