As our Ryan White Medical Case Manager, you will play a vital role in providing comprehensive psychosocial services to HIV+ patients referred by Legacy providers. Requirements

  • High School diploma or equivalent required.
  • One year of case management experience with people living with HIV and/or persons with a history of mental illness, homelessness, or chemical dependence is required.
  • All licensed professionals must maintain their professional licenses as defined by their respective Texas licensing/examining boards.
  • Excellent communication skills and customer service skills.
  • Working knowledge of community resources and treatment issues related to HIV.
  • Ability to manage multiple deadlines and prioritize.
  • Demonstrated competency with an EHRS system.
  • Ability to multitask in a fast-paced environment.
  • Above average organizational skills.
  • Strong typing and computer skills.

Nice To Haves

  • A degree in health, human, or education services is strongly preferred.
  • Bilingual (English/Spanish) preferred.

Responsibilities

  • Identify and screen HIV+ individuals referred by Legacy providers with social service needs.
  • Provide information, referrals, and assistance with linkage to medical, psychosocial, and basic needs services.
  • Advocate on behalf of clients to reduce service gaps and barriers.
  • Assist patients who have disengaged from care in reconnecting with medical services.
  • Comply with care standards, including DSHS Texas HIV Management Standards.
  • Complete Comprehensive Assessments and Individualized Care Plans according to program standards.
  • Maintain documentation and paperwork as required for program compliance.
  • Participate in training and community meetings as necessary.
  • Monitor the quality of services through regular evaluations with peers and supervisors.
  • Document services provided in EHRS client records within 72 hours.
  • Engage in Legacy's Performance Improvement and Quality Assurance Program.
  • Respect and uphold the dignity of all patients, families, visitors, and team members.
  • Collaborate effectively and foster positive working relationships within our team.
  • Travel between locations as needed to fulfill responsibilities.
