Through this comprehensive network, the organization serves more than 15,000 individuals annually across the Greater Toronto Area. This position assists the Executive Director in coordinating a high-quality nursing care service that complies with relevant government legislations, policies and standards, and reflects the organization's patient-centered approach, vision, and strategic direction. Ensures Hospice is the best provider of healthcare for hospice and palliative care. Promotes and delivers excellent patient care with effective management and leadership of high-performing teams. Intentionally develops a culture of safety, excellence, and participatory care as evidenced by meeting or exceeding operational and quality metrics in accordance with the organization's resident-centered approach, relevant government standards, residents' rights, and the organization's mission, vision, and philosophy.

Requirements

  • Registered Nurse with current Certificate of Competence from the College of Nurses of Ontario
  • Bachelor's degree in Nursing; or an equivalent combination of education and experience
  • Minimum 4 years nursing experience, including experience in Hospice, palliative care, Rehabilitation, Hospital or Long Term Care Nursing
  • Must be Hospice and Palliative Care Certified or obtain certification within two years of employment in the Hospice manager role
  • Current CPR Certificate
  • Strong leadership and administrative skills
  • Computer literacy required
  • Demonstrated experience in residential hospice and palliative care in accordance with the requirements of the role and applicable care standards.
  • Successful applicants must provide a recent satisfactory vulnerable sector police reference check and medical clearance certificate.

Nice To Haves

  • Proficiency in Cantonese and/or Mandarin is strongly preferred.

Responsibilities

  • Assists the Executive Director in coordinating high quality nursing services
  • Supervises patient care team in the delivery of patient/family care.
  • Supervises and assesses the quality of contracted services in relation to patient care.
  • Receives and assigns case referrals to personnel
  • Assist Executive director in ensuring that all clinical services for patient/family services comply with regulatory requirements and with professional standards
  • Participates in the development, implementation and evaluation of policies and procedures for patient/family services.
  • Participates in the selection and evaluation of all direct patient/family care staff.
  • Participates in the orientation and ongoing education of staff and volunteers
  • Coordinates the development, implementation and review of the interdisciplinary plan of care for each patient/family.
  • Communicates as needed with all members of the team, the patient/family, the medical director, the administrator, and others involved in the care of the patient to facilitate optimal care of patients/families.
  • Coordinates staff recruitment exercise to ensure adequate staffing to meet the needs of the Hospice patients
  • Participates in public relations and community activities that promote the organization's role as an effective member of the hospice system.
  • Ensures compliance of staff scheduling policies and procedures
  • Oversees Patient care of the hospice including monitoring, communication with patients, families and provide prompt intervention
  • Coaches, develops, evaluates and appropriately deploys staff to ensure a strong, effective and efficient team with good morale
  • Assesses training needs of the hospice staff and coordinates staff development programs, including orientation, in-service and certificate programs for the professional and nonprofessional nursing staff, to ensure a strong, effective and efficient team
  • Participates in quality improvement reviews especially related to the evaluation of nursing and contracted services.
  • Make recommendations for improving and enhancing the overall functioning of the Hospice
  • Coordinates and ensures accurate and up-to-date information management system for the Hospice
  • Implements effective risk management and quality improvement programs, including infection control, occupational health and safety, incident management.
  • This position will be expected to perform on-call professional duties
  • Works collaboratively with the other Managers in Yee Hong LTC
  • Keeps up-to-date with developments and best practices in the field of palliative and hospice care
  • Participates on committees and special projects and other duties as assigned.
