Role Description: At Oak Street, Population Health drives both our clinical and economic model and is focused on delivering high-quality, cost-effective care. As a Manager, Population Health, you will play a critical role in supporting our Oak Street Health patients, providers and business by designing and implementing key Population Health priorities. The Population Health Manager will work on programs and interventions, targeted to the right patients, to improve quality and cost of care. They will be an effective cross team leader, working side by side with Treehouse members and collaborating with providers, care teams, operations team members. Content areas for the Manager will include, but not be limited to, admission prevention, transitions-in-care, pharmacy and medication adherence, HEDIS / Stars, preferred partner relationships and medical cost management. This will require building relationships with our Treehouse team (including Data Science and Finance) and clinicians, including primary care physicians, nurse practitioners, pharmacists, registered nurses, licensed clinical social workers, and others. This is a fast-paced, high visibility role to ensure that we are driving innovation and improvement rapidly and consistently across Oak Street divisions, regions, and centers.
