About The Position

The Manager, Population Health will be responsible for managing a collaborative team-based strategy to manage complex patient services and initiatives, with a goal of achieving success in value based population health care outcomes. This role involves overseeing the day-to-day operations of a population health team, providing general oversight, management, and leadership. The manager will collaborate with executive leadership on strategies for population health services, pursue and manage collaborations with other organizational departments, vendors, and partners, and oversee data collection and evaluation of programmatic services. The position promotes population health initiatives within the team, the organization, and clients, and requires the ability to assess business/organization needs, compare benefit/value, and design/implement programs. It also assists in the development, planning, and implementation of a system approach to improving and maintaining health standards and health outcomes in identified populations. The role maintains communication and addresses the rapidly changing healthcare environment, leads a team to be flexible and responsive to organizational priorities and needs, and utilizes evidence-based practice in decision-making. A strategic and innovative thinker with proven ability to communicate and implement a vision and drive results is essential. Demonstrates management, organizational, and interpersonal skills, solves problems, and executes on initiatives. Creates an environment where population health programs are valued and embraced. Leads and manages a team to respond to organizational needs and provides population health-based services. Evaluates population health performance measures; sets and communicates standards for improvement.

Requirements

  • Bachelor's degree in healthcare related field required
  • Three years clinical and/or management experience in a healthcare and/or health plan setting is preferred.
  • Experience and demonstrated success in case management, disease management, wellness, population health, public health, and/or community health programs.

Nice To Haves

  • master's degree in public health or related healthcare area is preferred.
