About The Position

Description Responsible for the IV Sterile Manufacturing area that includes Chemo, TPN and IV preparations as well as the repackaging operation for unit dose dispensing. Also responsible for the EPA and hazardous waste for pharmaceuticals. Supervises the work activities of approximately one dozen pharmacists, six technicians and affiliating students. Responsibilities Roles & Responsibilities: Participate in daily multidisciplinary internal medicine hospitalist rounds Provide accurate and timely medication information to healthcare staff and patients Provide clinical and operational pharmacy support to pharmacy and medicine teams Collaborate with the Director and Pharmacy and Associate Director of Pharmacy to develop and implement clinical pharmacy programs and services in support of the pharmacy department mission Serve as an active participant in local and system committees in related clinical practice areas as assigned Actively participate in the Antimicrobial Stewardship program efforts Contribute to the Transitions of Care services and programs Identify, design, and analyze drug utilization reviews to promote rational drug therapy and determine action plan based on results Participate in departmental and hospital quality and cost-savings initiatives Sustain the hospital formulary, minimizing non-formulary procurement, utilizing therapeutic interchanges and promote rational drug therapy selection Work collaboratively with other health care providers and the clinical team members to develop and maintain a sound formulary system to support evidence based pharmaceutical care and cost-effective drug utilization. Coordinate formulary reviews, drug use evaluations, and quality assurance programs. Serve as a preceptor for pharmacy students and PGY1 pharmacy residents Develop and implement systems and procedures to maintain an efficient and effective workflow within the pharmacy and/or assigned areas Provide direction and guidance for the safe processing and dispensing of all medications for assigned area Address issues or services requiring intradepartmental coordination proactively Ensure an adequate inventory of pharmacy products and supplies to meet department demands Ensure the department is in compliance with all federal, state and local regulations, accrediting agencies; and Mount Sinai Heath System policies and procedures. Contribute to managing various personnel functions such as hiring, training, performance appraisals, coaching, disciplinary actions, and terminations. Exchange information and provide input for inter-and-intra departmental meetings Serve on a rotating basis, as an on-call administrator and troubleshoot issues that arise evenings, weekends, and holidays. Maintain adequate documentation to satisfy JCAHO requirements or other regulatory agencies Perform other duties as assigned Qualifications Requirements: Doctor of Pharmacy Degree required Current NYS Pharmacist license and registration ASHP-Accredited PGY-1 required, PGY-2 residency preferred Board certification (BCPS, BCCCP) strongly encouraged Hospital pharmacy and/or clinical experience; management experience preferred
