St. Camillus is seeking an experienced Hospice Clinical Car Manager to coordinate the day-to-day operations of our Hospice Agency and help ensure the delivery of quality, comprehensive care in accordance with federal, state, Joint Commission, and St. Camillus guidelines.

Requirements

  • Current Wisconsin RN license and current CPR certification.
  • Minimum of five years of hospice experience.
  • At least two years of supervisory experience, including coaching, evaluating performance, disciplinary action, and quality improvement.
  • Strong knowledge of hospice guidelines, regulations, coding, and reporting practices.
  • Excellent communication, presentation, organization, and leadership skills.
  • Valid driver’s license, and a driving record that meets St. Camillus Safe Driving Policy requirements.

Responsibilities

  • Lead, coach, and support Hospice clinical staff.
  • Coordinate admissions, daily nursing visits, case conferences, and care services.
  • Monitor quality outcomes, documentation, compliance, and reporting practices.
  • Serve as a resource and liaison for staff, physicians, allied health professionals, families, and campus partners.
  • Support budgeting, productivity standards, staff development, and ongoing training.
