About The Position
Sentara Health Plans is hiring for a Manager, Integrated Care Management- Nursing Facility/ HCBS / LTSS Waiver - Remote! This is a full-time, permanent position (40 hours) with standard working hours of 8am to 5pm EST, Monday through Friday. Remote opportunities are available in multiple states, with occasional onsite meetings in Virginia Beach or Richmond, VA. The role involves leading and developing a team of Care Managers supporting members in Nursing Facilities and Skilled Nursing Facilities. The manager will supervise 2 direct reports and provide oversight to 23 indirect team members, driving process improvement initiatives to enhance team performance and operational efficiency. Key responsibilities include monitoring quality outcomes, supporting the closure of member care gaps, preparing and presenting operational updates, metrics, and deliverables to leadership, and collaborating with providers, facility staff, and internal teams to improve member outcomes.
Requirements
- Bachelor’s Degree REQUIRED
- One of the following is REQUIRED: Registered Nurse License (RN) - Nursing License - Compact/Multi-State License or Licensed Clinical Social Worker (LCSW) - State license - North Carolina Social Work Certification and Licensure Board (NCSWCLB) or Licensed Clinical Mental Health Counselor (LCMHC) - State License - North Carolina Board of Licensed Clinical Mental Health Counselors (NCBLCMHC) or Licensed Professional Counselor (LPC) - State license - Virginia Department of Health Professionals (VADHP) or Licensed Professional Counselor (LPC) - State license - North Carolina Board of Licensed Clinical Mental Health Counselors (NCBLCMHC)
- 3 years of case management experience REQUIRED
- 1 year of leadership experience REQUIRED

- Extensive Nursing Facility or Long Term Care Experience preferred
- Has knowledge of Long-Term Services and Supports (LTSS) / Home and Community-Based Services (HCBS) or Medicaid requirements
