About The Position

The Manager Diabetes Program is responsible for the leadership, development, operation, and management of an American Diabetes Association (ADA)–certified diabetes program. Provides strategic direction and cross-functional leadership to ensure high-quality diabetes self‑management education and support services. Oversees Endocrine Nursing staff and Diabetes Educators. Collaborates closely with providers and interdisciplinary teams to deliver holistic, patient- and family-centered care across all age groups and diverse backgrounds. This role functions with a high level of autonomy while partnering with healthcare professionals to drive program excellence, staff development, and continuous improvement. The position promotes evidence-based practice, research participation, patient safety, and professional growth through ongoing education and engagement in professional organizations.

Requirements

  • Bachelor’s degree in nursing or health related field required; master's preferred
  • Minimum of 3-5 years progressive leadership experience required
  • Certified Diabetes Educator (CDE) and/or Advanced Diabetes Management (ADM-BC) Certificate required
  • Licensed Dietitian or Licensed Nutritionist by the State of MN required
  • Must have demonstrated leadership skills in team development, goal and objective setting, cross functional and matrix leadership and financial management

Nice To Haves

  • Outpatient medical clinic experience preferred
