Serves as a liaison between the patient/family and healthcare team for assessment, coordination of diagnostic work-ups, treatments and services, referrals for outside/contracted services, follow-up of study results, and documentation of the progress of their condition across the continuum of care. This position will focus on the coordination of a patient’s care for the patient population of pediatric diabetes, in collaboration with the entire healthcare team, over a prolonged period of time with transition into an outpatient setting. This position will also function as a member of the multidisciplinary CORE team in the outpatient clinic setting to provide/ensure nursing assessments, contribute to the plan of care, coordinate team conferences and provide patient/family education. Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. In addition to base pay, there may be additional compensation available for this role, including but not limited to, shift differentials, extra shift incentives, and bonus opportunities.
