Job Summary and Responsibilities Up to $5000 Sign On Bonus Offered Paid out in FULL after 30 days of employment! Location: Valley View Clinic - Council Bluffs, IA Schedule: Monday-Friday 8a-5p, No weekends, No holidays & No call! At CHI Health, we believe in a patient-centered approach to diabetes management. We're looking for an enthusiastic and knowledgeable RN (Nurse) who shares our commitment to making a tangible difference in the lives of those living with diabetes. What You'll Do: Conduct comprehensive individual education sessions for patients newly diagnosed with diabetes or those needing ongoing support. This includes, but is not limited to, medication management (insulin, oral agents), blood glucose monitoring, healthy eating, physical activity, and complication prevention. Serve as a primary point of contact for patient questions and concerns. Work closely with physicians, endocrinologists, dietitians, and other healthcare professionals to develop and implement individualized care plans. Conduct thorough patient assessments, identify learning needs, and develop tailored educational goals that are realistic and achievable. Monitor patient progress, adapt education strategies as needed, and celebrate milestones.

Requirements

  • Current Registered Nurse (RN) license that allows you to practice in our state
  • Graduate of an accredited nursing program
  • Basic Life Support (BLS) from the American Heart Association required
  • Certified Diabetes Educator (CDE) Certification required within 24 months of hire
