About The Position

Tap Healthcare Solutions is seeking an experienced Manager, Case Management – Trauma Services to join Memorial Hermann Health System in Houston, Texas. This leadership role is responsible for overseeing case management operations for trauma patients, ensuring coordinated care, efficient resource utilization, quality outcomes, and effective discharge planning throughout the continuum of care. The Manager will collaborate closely with physicians, nursing teams, social workers, rehabilitation professionals, and other interdisciplinary stakeholders to support patient-centered care and optimize clinical and financial outcomes. This position offers competitive compensation ranging from approximately $108,000 to $130,000 annually based on experience and the opportunity to lead within a nationally recognized healthcare organization.

Requirements

  • Case Management Certification (CCM, ACM, or equivalent) required.
  • Current and valid Texas Registered Nurse (RN) license or Licensed Master Social Worker (LMSW) credential.
  • Minimum of five (5) years of experience in one or more of the following areas: Case Management, Utilization Management, Discharge Planning, Care Coordination, Cost and Quality Management Programs.
  • Minimum of three (3) years of hospital-based nursing or social work experience.
  • Minimum of three (3) years of demonstrated leadership or management experience.
  • Strong knowledge of healthcare regulations, care coordination, utilization review, and discharge planning processes.

  • Trauma services, emergency services, or acute care case management experience.
  • Experience leading multidisciplinary case management teams.
  • Knowledge of value-based care models, population health, and quality improvement initiatives.
  • Strong communication, conflict resolution, and relationship-building skills.
