About The Position

Tap Healthcare Solutions is seeking an experienced Manager of Care Management – Social Work to join Providence Regional Medical Center in Everett, Washington. This leadership role is responsible for overseeing social work and care management services to ensure high-quality patient care, effective discharge planning, and coordination across interdisciplinary healthcare teams. The Manager will lead and support a team of social workers and care management professionals while collaborating with physicians, nursing leadership, and hospital administration to improve patient outcomes and operational efficiency. This position offers competitive compensation ranging from approximately $113,000 to $178,000 annually based on experience and the opportunity to contribute to a large, mission-driven healthcare organization.

Requirements

• Masters Degree in Social Work (MSW) from an accredited institution.
• Washington State Licensed Independent Clinical Social Worker (LICSW) license.
• Minimum of six (6) years of experience in case management within an acute care setting.
• Minimum of two (2) years of leadership experience at a manager or director level, preferably in case management within a large hospital.
• Strong knowledge of care coordination, discharge planning, and regulatory standards in hospital-based care management.
