About The Position

Community Health Network of Connecticut, Inc. (CHNCT) is currently seeking a Manager, Care Management- Rural Health for the Rural Health Transformation (RHT) Program. The Rural Health Transformation (RHT) Program is a federal grant that empowers states to strengthen rural communities across America by improving healthcare access, quality, and outcomes by transforming the healthcare delivery ecosystem.

Requirements

  • Four years post-secondary education required
  • Minimum 3-5 years in case management environment with increasing supervisory responsibilities is required.
  • Previous clinical experience with minimum 2 years direct clinical patient care is required.

  • Bilingual/bicultural abilities a plus.
  • Knowledge of CT Medicaid, HUSKY Health programs.
  • CCM preferred or within 3 years of hire.

Responsibilities

  • Under the direction of the VP, Care Management, responsible for daily operations of the Rural Health Transformation team.
  • In collaboration with senior leadership, assists in the development and implementation of programs and projects that support care management initiatives.
  • Functions as a clinical resource for staff, identifies and addresses training needs, and promotes consistent application of organizational policies and procedures across the team.
  • Reviews and monitors report dashboards related to referral management, operational metrics and performance outcomes to ensure achievement of key program objectives.
  • Reviews data analytics dashboard and manages the referral process.
  • Oversees staff assignments and coverage.
  • Accountable for performance reviews and corrective action plans for direct reports.
  • Collaborates with leadership and staff to identify opportunities for continuous quality improvement and supports the implementation of workflow and procedure enhancements.
  • Reviews case audit results to ensure standards are consistently met.
  • Facilitates team meetings and conducts focused training as needed.
  • Participates in intra/interdepartmental meetings, workgroups and external meetings/committees as needed.
  • Performs other duties as assigned.
  • Ensures compliance with DSS regulations, URAC standards and CHNCT policies and procedures.
  • Reviews policies and procedures annually and makes changes as necessary.
  • Monitors staff productivity and unit metrics assuring all metrics are met.
