About The Position

The Manager, Care Coaching serves as the primary point of contact for Care Coaches and is responsible for maintaining team compliance. You will also collaborate with other departments. It is our priority to maintain members in the least restrictive environment. In addition to improving member's health and well-being. Reports to Regional Care Coach AD. The Manager, Care Coaching employs a variety of strategies, approaches and techniques to manage a member's health issues. In this role you will support a team of 15-17 Care Coaches, be accountable to overall team compliance, coaching, and accountability models. You will support metric driven reports and identify barriers and supports to overcome them. You will maintain caseload ratios, while minimizing mileage and overtime. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through use of assessment, data, conversations with member, and active care planning. Decisions are typically related to resources, approach, and tactical operations for projects and initiatives involving own departmental area. Requires cross departmental collaboration and conducts briefings and area meetings; maintains frequent contact with other managers across the department. Must reside within 3 hours of Broward County.

Requirements

  • Bachelor's Degree in a health-related field
  • 2 or more years of managed care experience in Medicare and/or Medicaid.
  • 1 or more years of management/leadership experience.
  • 1 or more years of complex membership experience.
  • Proficiency in analyzing and interpreting trends
  • Progressive experience in the health solutions industry, with emphasis with leading and managing teams
  • Valid state driver's license
  • Proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits.
  • Self-provided internet service with at least a download speed of 25 Mbps and an upload speed of 10 Mbps.
  • Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Nice To Haves

  • RN or LPN with current State Licensure
  • Master's degree in human services, social sciences, or health related field.
  • Applicable State licensure in field of study
  • Proficiency in analyzing and interpreting financial trends
  • Previous experience in utilization management, discharge planning and/or home health or rehabilitation
  • Experience with health promotion, coaching and wellness
  • Health Plan experience
  • Previous Medicare/Medicaid Experience
  • Experience in maintaining NCQA standards.
