About The Position

Centerstone is seeking a compassionate, relationship-driven Managed Care Outreach Specialist to support individuals in accessing healthcare services, navigating insurance and managed care programs, and overcoming barriers that impact their health and well-being. In this role, you will serve as a vital link between patients, healthcare providers, managed care organizations, and community resources. You will proactively engage individuals who may benefit from healthcare services, connect them with appropriate programs and support, and help ensure they receive the care and resources needed to achieve better health outcomes. Requirements

  • High School Diploma or equivalent.
  • Two (2) years of experience in healthcare outreach, managed care, member services, patient navigation, community health, case management, customer service, or a related field.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Tuberculosis (TB) screening required for all direct care staff.

Responsibilities

  • Conduct outreach to individuals enrolled in or eligible for managed care, Medicaid, Medicare, and other healthcare programs.
  • Build trusting relationships with patients through phone, virtual, community, and in-person outreach efforts.
  • Educate individuals on available healthcare services, preventative care programs, and wellness resources.
  • Promote healthcare engagement by encouraging participation in appointments, screenings, and care plans.
  • Assist with outreach campaigns designed to improve health outcomes and care compliance.
  • Serve as a liaison between patients, providers, managed care organizations, and community partners.
  • Assist individuals in navigating healthcare, behavioral health, and social service systems.
  • Identify barriers to care, including transportation, housing, food insecurity, and access issues, and help connect patients with appropriate resources.
  • Support care management initiatives, including chronic disease management, preventative care, and patient education.
  • Conduct follow-up outreach to ensure individuals receive needed services and support.
  • Educate patients about healthcare coverage options and available managed care programs.
  • Assist individuals with eligibility determination, enrollment activities, and healthcare plan selection.
  • Help patients understand benefits, coverage options, and available support services.
  • Partner with managed care organizations to support continuity of care and member engagement initiatives.
  • Develop and maintain relationships with community organizations, service providers, and referral partners.
  • Connect individuals to community-based resources that support overall health and wellness.
  • Participate in health fairs, outreach events, and community engagement activities.
  • Advocate for patient needs and promote culturally responsive, person-centered care.
  • Maintain accurate documentation and data entry in accordance with organizational and regulatory requirements.
  • Track outreach efforts, enrollment activities, referrals, and patient outcomes.
  • Collaborate with multidisciplinary teams to improve patient engagement and health outcomes.
  • Participate in quality improvement initiatives, department projects, and other duties as assigned.
