About The Position

Malama leverages the best of technology and people to deliver exceptional care - we build novel biometric tracking and patient engagement tools that allow skilled Care Managers to provide high-touch care navigation and coordination for pregnant and postpartum mothers that improves health outcomes for mom and baby. We’ve raised $9.2M in funding to date and are on target to hit profitability early this year. Our mission is to improve pregnancy-related outcomes beginning with conditions that disproportionately affect historically underserved groups. We're seeking Malama Ambassadors to serves as a trusted connector between high-risk pregnant and postpartum Medi-Cal members and Malama! Embedded in WIC clinics, Federally Qualified Health Centers (FQHCs), and community-based organizations (CBOs) across the NorCal region, the Ambassador performs outreach, eligibility screening, and warm handoffs to Malama's Enhanced Care Management (ECM) Birth Equity services. This is a great entry point for recent graduates or early-career advocates who want to build a meaningful career in community health and birth equity. Requirements

  • Current Community Health Worker (CHW) certification OR equivalent community health experience
  • Deep ties to and lived experience in the communities served: Sacramento / San Bernardino / Fresno / Los Angeles / Alameda
  • Commitment to birth equity, reproductive justice, and culturally congruent care
  • Strong interpersonal skills and ability to build trust with diverse populations
  • Ability to travel regularly across assigned counties for partner site visits
  • Comfort with engaging with CBOs, following up, and tracking outreach activities

Nice To Haves

  • Doula training or certification (Malama will cover doula training if not already completed)
  • Bilingual or multilingual — Spanish, Tagalog, Vietnamese, or other languages spoken in priority counties
  • Prior experience with Medi-Cal, WIC, FQHC, or community-based maternal health programs

Responsibilities

  • Conduct in-person outreach at WIC clinics, FQHC waiting rooms, and CBO program sites across assigned counties
  • Build trusted relationships with pregnant and postpartum Medi-Cal members, particularly those who are Black, Indigenous, or from communities of color
  • Distribute multilingual outreach materials (Spanish, Tagalog, Vietnamese, and other languages per county demographics)
  • Represent Malama at community events including baby showers, health fairs, and partner-hosted convenings
  • Screen community members for Enhanced Care Management (ECM) Birth Equity eligibility per CalAIM criteria
  • Conduct warm handoffs to Malama's care coordination, doula, and CHW navigation teams
  • Coordinate with Kaiser Permanente care teams for KP Medi-Cal members to ensure complementary service delivery
  • Support member enrollment and onboarding into Malama's ECM Birth Equity program
  • Embed in partner site workflows at assigned WIC, FQHC, and CBO locations on a regular, scheduled basis
  • Collaborate with partner site staff to co-develop and refine ECM referral pathways
  • Participate in partner kickoff meetings, quarterly learning community sessions, and ongoing site check-ins
  • Support MOU implementation and documentation of Ambassador activities at each partner site
  • Document all member interactions, referrals, and outcomes in Malama's PointClickCare electronic health record (EHR) system
  • Track KP-specific metrics including NICU admission rates, ED utilization, and failed-to-keep (FTK) appointment data
  • Contribute to baseline, progress, and final reporting requirements under the CalAIM IPP grant
  • Participate in Malama team meetings, supervision, and ongoing professional development
